Step One

Brief consultation with Dani

​​​​Discuss your assessment goals, provide some basic background information, and ask any questions you may have about the assessment process. This is a free, no-obligation consultation.

Step Two

Schedule, legal documents, and payment

Choose the interview date and time that works best for you, electronically complete the required legal documents, and make your payment online.

Step Three

Forms

Complete the electronic questionnaires and assessment measures at your own pace.
All legal forms and empirically validated measures must be completed prior to the clinical interview.

Step Four

Clinical interview

Meet virtually with one of our providers for a 90 - 120 minute clinical interview using the MIGDAS-2

Step Five

Results + (optional) Report

Provider interprets and integrates the data from the empirically validated assessment measures, questionnaires, and clinical interview. Phone call within one week to discuss the results. Written reports are available within two weeks of completing the clinical interview.

Fees

Our fees are straightforward and transparent. Payment is due in full prior to the clinical interview. Please note: NeuroSpark is considered an out-of-network provider with insurance plans.

Includes: phone consultation with a team member, electronic questionnaires and testing, HIPAA-compliant video conference clinical interview with a provider, and phone call follow-up to discuss the results


Assessment + formal report - $1100 + $500
Includes: phone consultation with a team member, electronic questionnaires and testing, video conference with a provider, phone call follow-up to discuss the results, and formal written report that details the findings of the evaluation


Assessment + formal report + detailed feedback session with evaluator - $1100 + $500 + $250
Includes: phone consultation with a team member, electronic questionnaires and testing, video conference clinical interview with a provider, phone call follow-up to discuss the results, formal written report that details the findings of the evaluation, and 50-minute follow-up session with a provider to discuss the findings, recommendations, available supports, and your experience with the process and results

*A formal report may be purchased within 6 months of the assessment - $600


Wait Times

At NeuroSpark, we want to eliminate the barrier of long wait times and wait lists for assessments. We deliver results quickly. Here’s what you can expect:

  1. Consultation call with Dani - Typically within one week of your inquiry. Use Calendly to schedule a time that works best for you.

  2. Clinical Interview with Provider - Typically within two weeks of your consultation.

  3. A phone call to discuss results - Within one week of completing the clinical interview.

  4. Written report - Within two weeks of completing the clinical interview.

  5. Follow-up services - Can be scheduled any time following the completion of a clinical interview
